Short version
- We collect only what you choose to tell us — your name, your email, and what's going on in your practice.
- We use it to reply, and if we work together, to do the engagement. Nothing else.
- We don't sell it, don't run advertising against it, and don't track you across other websites.
- Ask us to delete your inquiry and we will.

Who we are
Forge Operating Partners LLC is an operations and advisory practice based in Arizona. We work with founder-led ABA and behavioral health practices on the processes, systems, and standards behind the clinical work.
This policy covers two things: the information this website collects, and the information we handle once a practice becomes a client. It was last updated on October 2, 2026.

What we collect
From the inquiry form. Your name and email are required. Your role, phone number, practice name, and where the practice stands today are optional — leave anything out you'd rather not share. What you write in the message box is yours to control.
Automatically. Like any website, the service that hosts this site records basic technical details about a visit — network address, browser type, which pages were requested. We use that to keep the site running and to stop automated abuse, including a limit on how many inquiries can be sent in a short period.
During an engagement. Whatever the work requires: org charts, staffing and compensation records, policies and procedures, financial summaries, vendor and payer information, and access to the systems we're helping you rebuild.
What we don't collect. This site runs no advertising trackers and no analytics services, and it stores nothing about you in your browser. There is no cookie banner because there is nothing to consent to.
How we use it
- To read your inquiry, reach out, and schedule an initial call.
- To deliver the work we've agreed on.
- To keep records we're required or reasonably expected to keep — agreements, invoices, correspondence.
- To protect the site from spam and automated submissions.
We don't build advertising profiles from your information, and we don't make decisions about you by automated means.
Health information and business associate agreements
Forge is not a healthcare provider and delivers no clinical care. We don't treat patients and we don't hold patient records of our own.
Where a practice engages us under a business associate agreement, we handle protected health information only as that agreement and the practice's instructions allow — usually in passing, while reviewing or rebuilding intake, scheduling, documentation, or reporting workflows.
We don't use protected health information for our own marketing, and we don't sell it. When an engagement ends, we return or dispose of it as the agreement requires, except where we're obliged to retain something.
If you're a patient or family member of a practice we advise, send your request to that practice. They hold the relationship and answer for your records. Where a business associate agreement and this policy differ on health information, the agreement governs.
Please don't send client or patient information through the inquiry form. Describe the problem instead — "intake takes eleven days" tells us what we need without putting anyone's records in an email.
How long we keep it
Inquiries sit in our email until we've replied and the conversation has run its course, or until you ask us to delete them — whichever comes first. Engagement records follow the client agreement and any business associate agreement. Anything we're required by law to keep, such as tax records, we keep for the period the law sets.
Your choices
Email justin@forgepartners.us and ask to see what we hold about you, have it corrected, have it deleted, or stop us contacting you. We'll confirm it's your request before acting, and we'll do it at no cost.
Two honest limits. We may keep a short note that you asked us to delete you, so we don't reach out again by mistake. And if your information is part of a client's records, we route the request to that practice — they decide, not us.
If you're in California
In the last twelve months we've collected identifiers (name, email, phone, practice name), commercial information (what you tell us about your practice and its revenue stage), professional or employment-related information (your role), and basic internet activity from hosting logs. We collect it from you directly and from our hosting provider, and we use it for the purposes described above.
We don't sell or share that information, and we don't use it for targeted advertising. Because the site runs no tracking, there's nothing for a "Do Not Track" browser signal to turn off. You have the right to know, access, correct, and delete, and you won't be charged less or served differently for using these rights.
How we protect it
The site is served over an encrypted connection. Access to inquiry email is limited to us. Submissions are checked for automation and rate-limited so the form can't be flooded. Nothing we do is advertised as unbreakable — no system is — which is why the form asks for the problem, not the patient files.
Children
This site is written for practice owners and isn't directed at anyone under 16. We don't knowingly collect information from children, and if we find we have, we delete it.
Changes to this policy
If this policy changes, the date at the top changes with it and the current version lives at this address. If a change meaningfully affects how we handle information you've already given us, we'll say so where you'd see it.
